Provides assistance to older adults to remain independent: Chore service; Respite Care, Transportation; Outreach, Health Clinic, Congregate Meals, Home delivered meals, Care Management, Education, Personal Care, Housekeeping, Health Promotion, Legal assistance, Family Caregiver Support, Emergency Response System, SHIIP Program, Ombudsman. (Service Area: Antelope; Boyd; Burt; Cherry/City of Valentine, Colfax, Dakota, Holt, Knox, Nance, Rock, Thurston, Boone, Brown, Cedar, City of Columbus, Cuming, Dixon, Keya Paha, Madison, Pierce, Stanton and Wayne counties)

Medicaid eligibility and waiver applications; Medicaid home and community based services, respite services, Commodity Supplemental Food Program, Social Services for Aged and Disabled Adults, assisted living services to allow persons who are 60+ or disabled to live independently; chore assistance, adult day program, home delivered meals, congregate meals, non-medical transportation; Aid to the Aged, Blind or Disabled (cash payment based on social security rules; however individual cannot receive SSI due to disability expected to last less than 12 month)s.
Provides assistance with Power of Attorney documents.
